Once a patient’s home care plan is developed… the PATIENT can choose the amount of time devoted to each caregiving task/service. Each Patient’s Home Care Plan is customised to the client’s medical conditions by a specialised home care team of nurses, caregivers, dietitians and certified special needs experts.
During the first 3 months the Client is matched with a lead Caregiver who works with the patient and home care team to ensure on-going long-term success.
Off Road Pit Stop Inc. is contracted out to provide medical pick up and delivery services for Self Directed Home Care Canada Inc. Many home care patients are unable to pick up their prescriptions. Not only does Off Road Pit Stop Inc. deliver on time but also ensures the patient is able to open the bottle and is taking the medication on a schedule.
